Most people leave the hospital with a diagnosis, a prescription, and a list of questions nobody fully answered.

What happens the day your supplies arrive? Who shows you how to use the pump? Is it normal to feel like you haven't quite settled into a routine by week two? (It is. And with time, it becomes much more manageable.)

This transition guide walks through what to expect from your first 72 hours home through your six-month check-in. What your care team is doing, what you’ll be learning, and when things start to feel like your new normal.

Fill out the form and we’ll send it right to your inbox. Someone from our team may follow up too to offer support and answer any questions that come up after reviewing the information.

What's Inside

Days 1–3
Before your supplies arrive at home, our team is coordinating insurance verification, reviewing your TPN prescription, and preparing your care plan so you're ready for a safe transition home.
WEEKS 1–2
Your supplies and pump arrive, with a nursing visit often within 24 hours. You'll learn to connect/disconnect, troubleshoot the pump, and manage line care — then nursing support continues as things start to feel routine and your care team keeps checking in.
MONTH 1
Things start to click. Your care team fine-tunes your lab monitoring and nutrition plan, while you become more comfortable with your daily routine. Support begins to shift from initial training to ongoing coaching and guidance.
